First notice of loss

Acord Notice of Loss including:

• Date of Loss
• Description
• Policy number
• Email/phone contact data insured

We must also have:

• Driver info
• Unit info
• Dash Cam
• Pictures of the scene

• Claimant Name
• Clamant Contact information
• Damage pictures
• Insurance info (when available)

 Regardless of the claim situation, make no statements or admissions to fault. Please take pictures of the claim scene and forward to our attention.  

 We need a statement from the insured, property manager, driver, etc. - of what happened 

 All policies have reporting conditions. Please educate yourself as an insured on these to avoid claim issues.
